New Clients

At Deify, we want you to focus on your treatments, not rushing in and filling out paperwork. In order to create a more pleasant experience for you, we ask that you please fill out the online Health History and Consent Form prior to your first appointment. That way you can come in, sit back, and relax. We ask that you arrive 15 minutes prior to your first scheduled appointment so that we can review your forms with you and with your service provider.
